The City of Bangor is announcing a new career opportunity in the Health and Community Service Department for a HIV Medical Case Manager. This is a temporary, 6-month position with potential opportunity for extension if needed. The hours are per-diem, 8 hours per week during the days/hours of Monday through Friday, 8am-4:30pm. This position will provide medical support to individuals with HIV in tandem with targeted medical case management to clients who are part of a local HIV cluster in the Bangor, Maine area. This person will work closely with a vulnerable population who may be unhoused or are experiencing substance use disorder. The population affected the most by the HIV cluster needs heavy advocacy and follow-through with medical care and connection to needed services. The duties of the position may include:
Provides education on HIV and associated medications.
Relays the importance of attending scheduled medical appointments and taking prescribed medication.
Answers client’s questions regarding HIV and medication in collaboration with their medical provider.
May, in the future participate in Directly Observed Therapy (DOT)of HIV medications.
Communicates and collaborates with medical providers, case managers, State public health nursing, State HIV program staff and community supports to coordinate medical care and to address social determinants of health.
Accompanies clients to medical appointments when needed.
Coordinates client transportation if needed.
Obtains all consents needed to discuss a client’s medical case with other providers adhering to HIPAA guidelines.
Makes referrals to community support as appropriate.
Requirements:
State of Maine registered nurse license.
State of Maine Driver’s License.
Active CPR certificate.
Ability to effectively communicate with associates and participants in a courteous, tactful and professional manner.
Knowledge of standard office practices and procedures, and ability to operate basic office equipment, including a computer and the telephone.
Basic knowledge of Human Immunodeficiency Virus (HIV).
Ability to create and maintain accurate records and files.
Ability to work independently.
Willingness to be trained in HIV Medical Case Management or HIV support.
Ability to work in various settings including outdoors
Difficulty of Work:
Incumbent performs a broad array of duties, with interrelationships among them not self-evident. Analysis and independent judgment is needed in selecting the appropriate guidelines, instructions or rules to apply to specific tasks.
Visiting outdoors with houseless individuals will be required when needed.
Responsibility:
Incumbent performs duties according to Maine State Board of Nursing professional standards of Practice and department policies and procedures, using independent judgment in making decisions having direct impact upon individuals diagnosed with HIV. Work is reviewed for overall soundness in judgment, general technical adequacy and conformance with practices and policy. Unusual problems or circumstances are discussed with supervisor at the incumbent’s discretion.
Incumbent ensures follow through with connections to services.
Personal Relations:
Incumbent maintains frequent contact with department associates, representatives of other governmental jurisdictions, non-profit agencies, vendors, patients, medical providers and members of the general public for the purposes of rendering clinic services.
