BE IT FURTHER ORDERED THAT the above processing fees shall be increased
annually on July 1st of each year beginning in 2004 by a percentage equal
to the annual percentage increase in the consumer price index for the
immediately preceding calendar year.
Fees shall be rounded up to the next highest dollar. The advertising reimbursement fees shall
also be increased annually on July 1st of each year beginning in
2004 by a percentage equal to any advertising increases.
